Physical Therapist (PT) - Sign on Bonus for Full Time (Healthcare)
Full Time, Part Time, and PRN Physical Therapist (PT) positions available.
Full Time Physical Therapists are eligible for a sign-on bonus after 6 months and a retention bonus after 12 months.
Responsibilities to include providing care in the following settings: Skilled Nursing Facility (SNF), Outpatient, and Home Health Care.

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing physical therapy services for patients in both skilled nursing facilities (SNFs) and home care settings.
Responsibilities of a Physical Therapist (PT)
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans
- Deliver skilled physical therapy services in accordance with physician orders, clinical guidelines, and regulatory requirements
- Treat a diverse patient population including post-surgical, orthopedic, neurological, and medically complex cases
- Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely in EMR systems
- Monitor patient progress and modify treatment plans as needed to achieve clinical goals
- Educate patients, caregivers, and SNF/home care staff on mobility strategies, safety, and therapeutic exercises
- Communicate and collaborate with physicians, nurses, occupational and speech therapists, case managers, and other healthcare professionals
- Ensure compliance with all federal, state, and company policies, including HIPAA and Medicare guidelines
- Participate in care team meetings, in-service training, and quality improvement initiatives
- Travel between SNF locations and patient homes as assigned
Qualifications of a Physical Therapist (PT)
- Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
- Current and valid Ohio Physical Therapist License.
