💰 Pay: $80,000-$100,000/year, DOE

Midtown Oaks Post Acute is seeking a detail-oriented and experienced MDS Coordinator to lead our facility's assessment and care planning processes. If you're passionate about quality outcomes, compliance, and resident-centered care, we'd love to have you on our team.

What You'll Do:

• Oversee the completion and accuracy of MDS assessments and care plans
• Ensure compliance with federal and state regulations
• Collaborate with interdisciplinary teams to support optimal care outcomes
• Track and manage Medicare and Medicaid reimbursement processes
• Educate staff on documentation and MDS processes

What We're Looking For:

• Active RN or LVN license in California
• Prior MDS experience required (minimum 1 year preferred)
• Knowledge of PDPM, RAI process, and regulatory guidelines
• Strong communication, organizational, and analytical skills
