We are seeking a full time Case Manager to join our fast growing home healthcare team!
Job duties will include: Communication and direct oversight in the daily activities of our field staff Care coordination of our patients upcoming needs, services and discharges Communication with patients on questions regarding services being rendered or to be rendered and changes to plan of care orders Educating the staff on documentation requirements and changes set by CMS and regulatory bodies Ensure patient charting is correct and completed timely to not interrupt patient continued care or billing practices Connecting clients professional services and assess their well being Evaluate our clients progress and make adjustment to improve the outcomes Follow up with discharged patients to ensure they are satisfied with services and still in good physical and mental health Assisting with additional duties when needed.
