Clinical Coordinator- Hematology/ Oncology (Full Time, Day shift) (Healthcare)



Job Summary:
Assesses, plans, facilitates and evaluates the coordination of patient care according to unit/department policies and procedures. Provides clinical leadership, utilizes effective problem solving strategies and applies evidence based practice to improve patient care. Works under the supervision of the Nursing Manager/Director.
 
Minimum Job Requirements:
  •  Registered Nurse Licensure within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) – must maintain active and in good standing throughout employment
  •  CPR American Heart Healthcare Provider – must maintain active and in good standing throughout employment
  •  PALS - American Heart Association - must maintain active and in good standing throughout employment within 1 year
  •  Aphon - Pediatric Chemotherapy and Biotherapy Provider Course– must maintain active and in good standing throughout employment within 1 year
  •  2-4 years RN experience
  •  Leadership experience

Essential Job Duties:
Nursing Care Functions:
  • Oversees & administers direct care to immune-compromised patients including: Central line/IV site monitoring, lab draws & monitoring during high census; performs nursing assessment & intervention.
  • Verifies patient Roadmap, protocol and orders for correctness and accuracy, upon admission.
  • Ensures completion of patient admission history, prepares and accurately completes the discharge planning process accurately as per department and hospital policies and procedures.
  • Plans, implements and documents the plan of care in collaboration with the other disciplines and services to assure safe and efficient care.
  • Follows physician's orders, administers chemotherapy & patient medications & reconstitutes medications when needed. Adheres to six rights of medication administration as per policy.
  • Provides interventions which include electrolyte infusions, sepsis monitoring, and transfusions with blood products.
  • Utilizes electronic innovations to provide efficient patient care and attends to patient call lights in a manner that promotes patient safety as per hospital policy.
  • Ensures safe analgesics/narcotics administration, monitoring and reassessment as per hospital Pain Management Policy and Procedure.
  • Ensures appropriate restraint orders, use and documentation and monitoring as required by Sedation Policy and Procedure.
  • Ensures proper and safe wound care as required by Wound Care Policy and Procedures.
  • Identifies opportunities to improve processes and assists with projects to enhance workflow utilizing Lean methodologies and tools.
  • Assists and supports the RNs and team with all hospital codes.
  • Responsible for patient care coordination on designated shift to include accountability for patient and outcomes.
 
Leadership Competency:
  • Coordinates and continuously monitors patient flow, supports patient throughout utilizing appropriate resources from admission, transfer or discharge and facilitates the processes in a timely manner.
  • Identifies clinical strengths & opportunities, re-assigns patient assignments to support clinical competency, coaches and mentors employees to improve performance and clinical outcomes.
  • Flexes the employees to align with census fluctuations. Collaborates with the Operations Administrator, Nursing Manager, and Nursing Staff Office to meet hospital needs.
  • Supports departmental goals and the hospital strategic plan. Actively participates in the nursing governance council and unit based committee.
  • Assists in communication of hospital updates and Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) Patient Perception Survey data.
  • Fosters staff empowerment and open communication. Leads and guides staff to build a high performance and engaged team.
  • Provides mentoring, coaching and performance feedback on an immediate basis to ensure the quality of nursing care.
 
Patient and Family Communication:
  • Ensures the provision of education about disease processes & treatment, and documents patient/family teachings on health related needs in the Electronic Health Record.
  • Works collaboratively with all levels of the hospital interdisciplinary team and promotes the team concept within department as well as hospital-wide.
  • Interacts with patients and families to review plan of care and solicits input and communicates any patient and family concerns to physician.
 
Patient Care Rounds and Updates:
  • Collaborates with a multi-disciplinary team in daily care planning rounds as appropriate while fostering a Family Centered Care environment.
  • Communicates the patient's progress to physicians including critical findings and changes in the patients' condition.
  • Oversees, implements and documents the plan of care in collaboration with other disciplines and services to assure care.
 
 
 
 
Charting/Documentation:
  • Actively participates in the performance review process through chart reviews. Ensures all unit documentation is complete and validates timeliness of charting and all requirements are met.
  • Participates in collection of all regulated unit based audited required by FACT, COG and COC
Quality and Safety:
  • Achieves a 95% CEI by conducting chart audits utilizing the EMR to ensure staff adherence to regulatory requirements of the DNV, CMS, DOH, FACT, COG, COC and other accrediting agencies. 
  • Achieves and maintains acceptable standards with infection control goals achieving acceptable designated target/rates by performing infection prevention standards for assigned shift.
 
Common Duties:
  • Maintains professional accountability by ensuring compliance thru timely completion of licenses, certifications, competencies, health requirements (PPD, Flu), initial competencies & education reqs.
  • Meets the Service Excellence Index (SEI) standards and complies with attendance guidelines and procedures and ensures behavior is compliant with standards.
Knowledge/Skills/Abilities:
  • BSN degree or currently enrolled in BSN program
  • Pediatric RN experience in Hematology/Oncology acute care setting preferred
  • Possesses proficiency in computers and Electronic Medical Record (EMR) preferred
  • Career Development: National certification Certified Pediatric Nurse i.e.; (CPHON) or (CPON) or (BMTCN) preferably within 5 years of hire
  • Ability to oversee and optimize the flow of department and provide exceptional patient care in a fast-paced environment
  • Able to analyze and interpret data and utilize the information to make judgments regarding patient care 
  • Effective verbal and written communication skills
  • Ability to positively influence, hold self and others accountable
  • Works effectively in collaborative environment
  • Ability to apply principles of evidence-based practice, analytical thinking and service excellence in practice
  • Able to provide coverage for on-call assignments per departmental and hospital policy
